Join us for an in-person gathering that promises connection, inspiration and grounding in our shared commitment to compassionate, equitable care for those facing serious illness. The SWHPN Annual Forum is designed specifically for hospice and palliative care social workers, providing unique, high-quality professional development while fostering a deep sense of community.

Set at the University of Minnesota in the vibrant city of Minneapolis, this year’s forum embodies SWHPN’s commitment to community-driven impact and support for social workers dedicated to compassionate care.

The 2025 theme is “Grounded in Values, Growing in Care.” We are looking for presentations that reflect the core values of social work and their influence on leadership, policy and advocacy, teaching, research and clinical practice in hospice and palliative care.

The Annual Forum (formerly called the General Assembly) will take place in Minneapolis, Minnesota, on April 28-29, while our Virtual Summits will occur online from August to December 2025.
